UPDATE: How to use Wurst with NeoForge
NeoForge 1.21.1
- Add the latest Wurst version for Minecraft 1.21.1 to your mods folder (link)
- Don't add Fabric API
- Instead, add the latest Forgified Fabric API for 1.21.1 (link)
- Add Sinytra Connector for 1.21.1 (link, pick the latest version that isn't
beta.11 or beta.12)
- Install NeoForge (no need to install Fabric)
Forge/NeoForge 1.20.1
- Add the latest Wurst version for Minecraft 1.20.1 to your mods folder (link)
- Don't add Fabric API
- Instead, add the latest Forgified Fabric API for 1.20.1 (link) (it says Forge, but works with NeoForge too)
- Add Sinytra Connector for 1.20.1 (link) (it says Forge, but works with NeoForge too)
- Install either Forge or NeoForge (no need to install Fabric)
Important: If you find older forum posts telling you to downgrade Wurst, Forgified Fabric API, or Sodium, ignore that advice! Those compatibility issues have been fixed and the latest versions work fine now.
Also Important: Sinytra Connector versions 2.0.0-beta.11+1.21.1 and 2.0.0-beta.12+1.21.1 are super broken. For now, you can fix this by downgrading to version 2.0.0-beta.10+1.21.1. https://modrinth.com/mod/connector/version/2.0.0-beta.10+1.21.1
For anyone reading this far in the future, if there is a beta.13 or higher version of Connector, try that first before downgrading anything! Check here to see if that exists.

Technically Wurst MC1.20.1 with Sinytra connector can work in both Forge and NeoForge, but in 1.21 I don't think there is currently a way to do that.
I also haven't heard of any NeoForge hacked clients yet.
